Job Description
Were seeking a dedicated and experienced Senior Occupational Therapist to join our skilled nursing facility in Hornell NY. In this role youll play a crucial part in enhancing the quality of life for our diverse geriatric and subacute populations through comprehensive therapy services.
Key Responsibilities
Develop and implement personalized occupational therapy treatment plans
Conduct thorough patient evaluations and assessments
Participate in family care plan meetings and interdisciplinary team collaborations
Monitor and adjust treatment plans to ensure optimal patient outcomes
Maintain accurate and detailed patient records
Assume Director of Occupational Therapy responsibilities during absences as assigned
Mentor and guide junior therapy staff members
Requirements
Masters degree in Occupational Therapy from an accredited institution
Current New York State Occupational Therapy license
Minimum 3 years of experience in geriatric and subacute care settings
Strong knowledge of evidencebased OT practices for elderly populations
Excellent communication and interpersonal skills
Ability to work collaboratively in a teamoriented environment
