Description

JOB SUMMARY: Under the supervision of the Home Care Manager the Home Care Assistant provides general and specialized administrative support to the Home Care department and orders home care supplies and durable medical equipment (DME).

ESSENTIAL DUTIES & RESPONSIBILITIES:

  1. Answers screens and transfers phone calls and emails.
  2. Prepares written correspondence including authorization letters.
  3. Performs general clerical duties including data entry faxing and mail distribution.
  4. Creates and maintains electronic filing system.
  5. Schedules and coordinates meetings for administrative supervisors.
  6. Orders supplies (incontinence supplies for participants and PPE equipment for inhome staff) and maintains office supply inventories.
  7. Reports any significant participant changes in condition to the Home Care Manager.
  8. Follows agency policies for incident reports grievances and appeals.
  9. Communicates effectively with participants family members caregivers staff and visitors.
  10. Completes documentation accurately and in a timely manner.
  11. Complies with safety policies and procedures identifying and immediately reporting any potential or actual unsafe acts or conditions per the policies and procedures. Takes necessary measures to ensure a safety environment for oneself coworkers contractors participants visitors and others.
  12. Monitors personal safety and attends to good body mechanics. Ensures infection and exposure control and is prepared in case of emergency.
  13. Continually seeks better ways for delivering services and communicating with participants.
  14. Demonstrates respect for and promotes participant rights including dignity selfdetermination access to care confidentiality and independence.
  15. Understands the importance of community involvement and participates as appropriate in activities that link Redwood Coast PACE to its communities.
  16. Effectively collaborates with staff peers and contractors to meet Redwood Coast PACE goals and further success.
  17. Complies with all policy and procedures.
  18. Demonstrates proficiency in delivering agespecific care.
  19. Consistently meets or exceeds quality assessment and performance improvement targets.
  20. Participates in the process of teaching training and mentoring student interns and volunteers working within the program area.
  21. Participates in oncall coverage in conjunction with the Home Care team.
  22. Acts in a manner consistent with agency values.
  23. Other duties as assigned.

Qualifications

EDUCATION &/or EXPERIENCE:

  1. High school diploma or GED equivalent required.
  2. At least two 2 years of customer service and/or office experience.
  3. At least one 1 year of experience working with a frail or elderly population preferred.

LICENSES OR CERTIFICATES:

  1. Licenses or certificates are not required for this position.
  2. Valid California Drivers License and personal auto liability insurance preferred. Personal vehicle in good driving/operating condition insured for State minimum liability requirements that can be used for the applicable job functions noted above preferred.

PRESCREENING REQUIREMENTS:

  1. Requires clearance of a DOJ and FBI criminal history background check
  2. Preemployment physical and PPD skin test.
  3. Requires Motor Vehicle Report verification.
  4. COVID19 and Influenza (flu) vaccines preferred.

JOB SKILLS:

  3. Works effectively with staff in the department to which assigned other departments and others such as participants family members and contracted providers.
  4. Courteous and professional communication.
  5. Highly organized.
  6. Effectively utilizes Microsoft Office and other relevant computer programs.

  8. Maintains excellent attendance and punctuality records.
  9. Work in a constantly changing environment with individuals of various backgrounds and abilities.

SUPERVISORY REQUIREMENTS:

  1. This position has no Supervisory requirements.

PHYSICAL REQUIREMENTS:

  1. Physical abilities sufficient to move between different staff work areas communicate with coworkers and the public operate a computer produce reports talk on the telephone and travel to other agency worksites.
