POSITION SUMMARY:

The Program Coordinator (PC) position at 360 Behavioral Health Support Services is a supervisory role that coordinates Direct Support Professionals (DSP) clients and their families Regional Center the Company and the Local Office. Their role is to provide oversight and support to their DSPs out in the field to ensure that service objectives are met with a high degree of quality that the family continues to receive services from Regional Center and that the services are being provided in a collaborative fashion amongst all parties. The Program Coordinator effectively manages their caseload to align with the companys standards and expectations. They will also be expected to provide any necessary support to ensure the sustainable and efficient operations of their Local Office.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

  • Lives the companys business theory of customer focus total participation continuous improvement leadership and mutual learning.
  • Ensures 85 of clinical direct services are delivered monthly.
  • Ensures comprehension implementation and reinforcement of the companys Zero Tolerance Policy in the region.
  • Ensures comprehension implementation and reinforcement of Health Insurance Portability and Accountability Act (HIPAA) in the region.
  • Ensures clients under their supervision fulfill billable services with minimal cancellations makeup or collaborating with staff and clients to uphold scheduled hours and coordinate make up hours.
  • Ensures program designs and behavior support plans are being met in collaboration with regional center clients and any behavioral agencies.
  • Provides clinical consultation including how to follow behavior plans and programming to clinical staff.
  • Delivers timely written program summaries and progress reports for each individual case before the deadlines set by the supervisor.
  • Conducts staff meetings and professional development meetings as indicated.
  • Offers direct services as necessary to enhance supervision effectiveness.
  • Seeks consultations and implements feedback provided by Department Manager regarding DSPs and clients.
  • Ensures services meet the needs of the families and escalates parent/caregiver concerns to the Department Manager.
  • Conducts and monitors assessments assigned from intake to start of services.
  • Attends client clinical meetings as needed.
  • Keeps consistent telephone/email communication and ensures a reliable method of recording scheduled appointments aiming to return phone calls/emails within 24 hours.
  • Provides supervision and quality assurance on their cases.
  • Provides updates and reporting SIRS to Regional Centers by mandated timelines.
  • Conducts utilization meetings with families and DSPs.
  • Assists as needed in interviews job training and intakes.
  • Actively attends and participates in all office and leads meetings.
  • Manages DSPs assigned to cases by providing guidance support training and coaching.
  • Completes all duties and/or tasks assigned by leaders and/or the business.

NON ESSENTIAL FUNCTIONS AND RESPONSIBILITIES INCLUDE BUT ARE NOT LIMITED TO:

  • Manages the mentorship program for Direct Support Professionals.
  • Participates in the local office staff vendor community and recruitment events.
  • Attends regional meetings and training.
  • Supports the office by taking on the role of an outreach recruiter to assist in networking for new employees.

QUALIFICATIONS

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge skill and/or ability required.

  • Ability to collaborate with a multidisciplinary team.
  • Excellent clinical and leadership skills.
  • Fluency with all Microsoft Office programs.
  • Selfmotivated responsible honest articulate excellent time management skills and skills to work independently.
  • Professional demeanor wellorganized and attention to detail.
  • Skills consistent with serving culturally and linguistically diverse families in low and high socioeconomic status areas.
  • Comfortable working in homes (studio apartment housing project etc. of families with limited resources.
  • Demonstrate effective communication skills to interact with clients family members and agencys staff.
  • Maintains reliable transportation and current liability insurance for their vehicle.

MINIMUM REQUIREMENTS:

EXPERIENCE

  • 1 years of clinical experience providing services to individuals with diverse diagnoses or disabilities.
  • 1 year of experience supervising staff in a clinical setting.
  • Bilingual preferred

EDUCATION:

  • High School Diploma or GED
  • Pursuance of bachelors degree

LICENSE AND CERTIFICATIONS

  • Ability to pass background check/Live Scan required.
  • TB clearance
  • Crisis Prevention Certification
  • CPR First Aid Certification

LANGUAGE SKILLS

  • Ability to read analyze interpret and implement policies and reports.
  • Ability to effectively present information and respond to questions from all levels of staff.
