Job Location:Our office is located in East Orange NJ

Job Responsibilities:

As a HomeSafe Therapist/Clinician you will play a vital role in supporting families dealing with stressors like homelessness substance abuse mental health concerns and child separation due to these stressors. Your responsibilities include:

Programmatic and Service Model Mastery:

  • Prompt Client Outreach: Respond promptly to client requests for phone screens and intake assignments.

  • Accurate Assessment: Assess clients presentations needs and strengths accurately.

  • Timely Intake Assessments: Complete intake assessments promptly.

  • Engagement Planning and Contracting:

  • Collaborative Planning: Develop a care plan in partnership with clients incorporating their needs and priorities.

  • Progress Monitoring: Regularly evaluate progress and renegotiate goals for growth.

  • Ensure FollowUps: Follow up on missed or canceled services.

  • Ongoing Reassessment: Renegotiate planning at regular intervals to address evolving client needs.

  • Clinical Excellence:

  • Effective Intervention: Utilize appropriate interventions both in noncrisis and crisis situations.

  • Empathy and Understanding: Create a judgmentfree empathetic space for clients respecting their feelings and personal presentation.

  • Tailored Approaches: Adapt your approach to meet client needs whether its individual family group or crisis intervention.

  • Medication Management: Consult with psychiatrists monitor medication regimens and attend to medication appointments.

  • Case Management: Provide case management services as needed.

  • Collaborative Care: Advocate coordinate and maintain contact with other providers involved in clients care.

  • TraumaInformed Care: Incorporate traumainformed care models.

  • Discharge and Termination:

  • Goal Achievement: Identify when therapeutic goals have been met and work toward appropriate discharge.

  • Client Preparation: Discuss progress with clients and facilitate timely discharge planning.

  • Transfer Assistance: Facilitate transfers/referrals for further services when appropriate.

  • Supervisory Consultation: Consult with clinical and program supervisors before discharge.

  • Paperwork and Planning: Complete termination paperwork and discharge planning promptly.

  • Documentation Meetings and Productivity:

  • Accurate Documentation: Complete documentation following agency and program guidelines.

  • Collaborative Meetings: Facilitate meetings and collaborate with referral sources as needed.

  • Attend Team Meetings: Actively participate in internal team meetings and case presentations.

  • Meet or Exceed Productivity Targets: Maintain or exceed program productivity requirements.

Transportation Responsibilities:

  • Vehicle Safety: Maintain a professional and safe environment in agency vehicles.
  • Child Safety: Ensure the safety of children and adults during transportation.
  • Vehicle Maintenance: Monitor and document vehicle maintenance regularly.
  • Log Maintenance: Keep required vehicle logs and maintain records.
  • Transportation Coordination: Prepare for pickup and dropoff of clients for program services.
  • Clear Communication: Communicate effectively with parents caregivers and children during transportation.

Qualifications:

To excel in this role you should possess:

  • Valid NJ State Drivers License in Good Standing.
  • Masters Degree in Social Work and a NJ State License such as LSW LPC orLMFT.
  • Two years experience working with children and families especially in child welfare.
  • Experience with diverse populations in an urban setting.
  • Knowledge of mental health various treatment modalities and community resources.
  • Crisis Intervention Skills: Ability to apply appropriate interventions in crisis and noncrisis situations.
  • TraumaInformed Care Knowledge: Familiarity with interventions for families experiencing abuse or trauma.
  • Organizational Skills: Solid organizational and time management skills.
  • Team Player: Ability to work well both independently and as part of a team.
  • Navigation Skills: Ability to support clients in navigating multiple care systems.

Work Environment/Physical Demands:

The role may involve:

  • Frequent Standing and Movement
