REPORTS TO: Coordinator of Veterans and Peer Services Integrated Behavioral Health Services (IBHS)
BROAD FUNCTION: Engage and Support Certified Community Behavioral Health Clinic (CCBHC) Clients in Various Stages of Substance Use and or Mental Health Treatment Services

IV. POSITION SPECIFIC RESPONSIBILITIES:
1. Works collaboratively with and provides input as part of multidisciplinary IBHS Team to support CCBHC clients.
2. Identify engage motivate and support clients within various levels of recovery.
3. Supports individuals in the CCBHC program with mental health substance use and related matters.
4. Provides case management support to CCBHC clients in recovery.
5. Serve as a role model for individuals and their families by discussion of experiential knowledge and recovery skills.
6. Coach individuals and their families about recovery regarding variations in coping styles motivations strengths and the resources of the individual.
7. Possess knowledge of community resources and public assistance programs and ability to assess/refer/link to those services.
8. Identify navigate and enhance motivation for engagement in various selfhelp and community recovery organizations.
9. Provides appropriate followup to best support the individual or familys needs and ensure linkage to appropriate recovery services.
10. Advocates on behalf of and with clients and family regarding expressed interest and preferences.
11. Collaborates with community services and provides referrals for additional services.
12. identifies risk reduction strategies and support each in positive steps forward.
13. Support clients in making self determined and selfdirected choices about their recovery pathway.
14. Maintains required documentation of services provided and coordinated including documentation in client Electronic Health Record (EHR).
15. Attend all meetings trainings and workshops as required by program.
16. Perform other duties as needed/required.
SPECIFICATIONS:
Education:
Required: HS Diploma/GED Equivalent
Preferred: Certified Recovery Support Professional (CRSP)
NOTE: If candidate is not CRSP actively working toward CRSP will be condition of employment.
Experience: Minimum of one year as a Peer support professional. Personal experience with the Mental Health and/or Substance Use treatment.
Knowledge: Knowledge of community resources social service and mental health systems.
Skills and Abilities: Ability to assess clients needs; aid in obtaining concrete services and the ability to complete all paperwork in a timely manner. Good written and oral communication skills. Ability to create a warm caring nonthreatening therapeutic atmosphere. The willingness to work with individuals from diverse backgrounds. An understanding of professional ethics and issues of confidentiality. The ability to work on a multidisciplinary team.
Work Environment/Physical Demands: The individual must possess the ability to perform work that requires frequent standing bending reaching squatting kneeling moving and lifting of up to or in excess of 50 pounds.
