Nurse Practitioner Geriatric Primary Care


SCHEDULE:

COMPENSATION:
$125000 to $155000 Annual Income
$105000 to $125000 Base Salary
$20000 to $30000 per Year in RVU Bonuses

LOCATION:
Akron Ohio

This practice sees patients in skilled nursing facilities (SNFs) assisted living facilities (ALFs) independent living facilities (ILFs) and private homes. They are currently hiring in the Akron area and will work with providers during the interview process to ensure a mutuallyagreeable geographic territory in which all patients / buildings will be located. There are options for facilityonly work if the provider prefers not to see patients in private homes.


COMPANY PROFILE:
This organization is dedicated to delivering convenient highquality primary care to geriatric patients in the comfort of where they call home. They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a providers office. Their services include among others primary care chronic care psychiatric care postacute care transitional care and remote patient monitoring.


POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients.

All patients are located in facilities or private homes. Depending on the territory the practice is able to offer different mixes of facilities (ALFs / ILFs / SNFs) and homebased patients including panels that do not include house call patients and are facilityonly. During the initial phone interview the practice will look at the providers location and discuss this in detail.

In SNF settings (with no travel during the day) providers see an average of 15 to 16 patients per day.

When traveling between ALFs ILFs and/or homes providers see an average of 9 to 12 patients per day (this is adjusted based on actual travel time and number of locations).

Specific duties of the role include but are not limited to:

conduct health assessments to identify patient needs
develop personalized care plans
regularly evaluate patient conditions and adjust treatment plans as necessary
administer medications treatments and other therapeutic interventions as prescribed
perform minor procedures and wound care as required (and as settings permit)
monitor vital signs and patient responses to treatment
educate patients and families on health management disease prevention and treatments
provide guidance on lifestyle modifications and selfcare practices
work with physicians nurses and other professionals to coordinate comprehensive care
ensure compliance with all relevant healthcare regulations and standards

The practice uses the eClinicalWorks EHR.

Oncall is phone triage only with very low volume. Providers rotate the oncall responsibility with a shift every 10 weeks (often less) for fulltime providers.



REQUIREMENTS:
Ohio NP license
DEA
advanced practice experience preferred not required
new grads with RN backgrounds in home health trauma or ICU are encouraged to apply
