We are hiring for a Mobile Crisis Team Co Therapist in Cullman AL.
Shifts Available:
- Full Time MondayFriday
- 2nd Shift 3:00 PM to 11:00 PM
- 3rd Shift 11:00PM to 7:00 AM
- Part time Weekends
- 7:00 AM 7:00 PM
- 7:00 PM 7:00 AM
What we are looking for:
- Bachelors in Human Service Field
- Prefer experience working with cooccurring illness
- Must attend Case Management Training
- Valid Alabama drivers license
- Good 5year motor vehicle record
- Reliable transportation
- Minimum of $30000/$100000/$50000 personal automobile liability insurance
- Excellent oral and written communication skills
- Ability to work nontraditional hours/days
What youll be doing:
- Help individuals experiencing a crisis event to experience relief quickly and to resolve the crisis situation when possible.
- Visit clients in their environment to assess needs
- Determine the level of risk faced by the individual in crisis and assess the most appropriate response to meet the need
- Provide appropriate care/support while avoiding unnecessary law enforcement involvement ED use and hospitalization.
- Connect individuals to facilitybased care as needed through warm handoffs and coordinating transportation when and only if situations warrant transition to other locations.
- Track the status and disposition of linkage/referrals for individuals needing intensive service levels including:
- where the client is currently located
- how long they have been waiting
- what specifically is needed to advance them to service linkage or requirements for service approval and transport
- Be able to schedule intake and outpatient appointments for individuals in crisis 24 hours a day 7 days a week to facilitate rapid access to outpatient service and continuity of care
- Collect information daily on bed availability in regional crisis residential and inpatient psychiatric facilities to speed placement.
- Follow up on referrals from: other WellStone staff law enforcement dispatch operators and other partnered agencies within a 24hour period
- Facilitate Treatment Team and staffing sessions
- Provide comprehensive clinical assessment
- Complete documentation within 24hour period
- Initiate petitions and testify in court as necessary
- Facilitate psychoeducation for families
- Complete scheduled trainings as required by center and DMH
- Advocate on behalf of client with community stakeholders
- Collaborate with intradepartments and outside agencies
- Perform crisis intervention and resolution
- Refer clients to next level of care
- Provide mentoring to cotherapist in a positive constructive manner
- Ensure compliance with internal and external administrative codes
- Transport clients to center and community resources
- Complete other duties as assigned by Outreach Services Coordinator/executive team
