The Nursing Aide provides direct patient care. Assignments will be clinical in nature and completed under the daily direction of a licensed nurse to include direct patient care activities such as taking vital signs performing EKGs and assisting patients with the basic activities of daily living. In addition the Nursing Aide will be assisting with therapeutic treatment regimens under the supervision of a licensed therapist.Direct Patient Care Activities: Takes vital signs including blood pressure documents and reports to nurse. Takes and transmits EKGs Assists with activities of daily living using adaptive equipment as specified by therapist. Attends to patients nutritional status by measuring height and weight assisting with eating monitoring intake and output calorie counting and monitoring patients appetite. Performs and records Glucometer blood sugar readings correctly and accurately. Application of telemetry electrodes and unit preparation under direction of a licensed nurse. Attends to patients elimination needs by assisting with bed pan urinal toilet and commode. Assists in the positioning of patients including turning and maintaining proper body alignment. Applies elastic stockings as directed. Utilizes basic observation skills to obtain patient information reports changes in patient condition to RN/LPN and/or specific therapist. Unit Support Activities: Communicates and documents accurate and concise information pertinent to patient care on the intake and output sheet nursing care flow sheet pain flow sheet restraint/seclusion checklist and calorie count record. Identifies and requisitions supplies needed for patient care when appropriate. Obtains specimens from following list labels and readies for lab processing: Clean voided urine Guiac testing of stool 24 hour urine collection Transports patients to Radiology and assists Radiology Technician as needed. After successful completion of educational training and under the supervision of therapist and supervisor may assist with: Application of hot and cold packs Wheelchair positioning adjustment/modifications. Donning/doffing orthotics prosthetics and braces. Transfers of patients to chairs wheelchairs with or without the use of assistive/transfer devices. Supervising and/or assisting patients with swallowing disorders at mealtimes including mixing fluids for appropriate diet consistencies. CPM machine on and off after initial placement by therapist.
