POSITION OVERVIEW

We are seeking a Connections Specialist to assist the Connections Manager with the daily operations of the Admissions department. The primary responsibilities include promptly responding to calls voicemails and emails from individuals seeking treatment at MHFV. Additionally the role involves conducting interviews with prospective participants and collecting the necessary information for their admission.
ESSENTIAL JOB RESPONSIBILITIES
- Complete Initial Screenings of all MHFV programs in accordance with agency terms and funder agreements.
- Ensure the Connections Department and all operations are conducted in compliance with all funding sources and grantors (DBHDD CARF SAMSHA VA & HUD etc. processes.
- Monitor electronic records associated with each connection weekly to ensure all information is entered completely & accurately.
- Maintain knowledge of MHFV connection & clinical policies ensure compliance & adherence and updates them as needed.
- Conduct outreach and connect women to program services.
- Manage MOUs with all collaborating partners.
- Identify resources that will support and enhance collaborative partnerships.
- Adhere to measurable performance goals that include documenting the number of agencies visited the number of women referred the number of women screened for eligibility and the number of unduplicated participants enrolling in program services.
- Conduct site visits to treatment services organizations and work with wraparound services providers.
- Ensure compliance with all regulatory and organizational policies and procedures.
- Participate in team meetings and contribute to the continuous improvement of the admissions process.
MINIMUM QUALIFICATIONS.
- Bachelors degree in Social Work Counseling or Human Services with experience in a substance abuse treatment environment preferred. CARES/CPS certification is also acceptable.
- Tech savvy; Proficiency in Microsoft Office Suite and other relevant software.

SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
