Bachelors Degree and an interest in healthcare improvement of healthcare processes and the use of technology to support clinical care.
Experience with MS Office applications including Word Excel PowerPoint and Access preferred.

General Overview:
The Chart Reviewer (CR) will support the National Committee for Quality Assurance (NCQA) PatientCentered Medical Home (PCMH) Recognition program for Partnersaffiliated ambulatory care practices. Principal duties will involve reviewing electronic health records (EHRs) preparing presentations participating in and presenting at onsite practice meetings with practice consultants to review chart review results with ambulatory practices and providing educational guidance and support related to documentation requirements to ambulatory practices. Access to a vehicle and a valid drivers license is required. The position requires attention to detail strong communication and organizational skills. A 12 year commitment is preferred.
Responsibilities:
Conducts chart reviews of online electronic medical records to assess compliance with NCQA PCMH documentation requirements.
Prepares practice scoring sheets to report chart review results.
Educates ambulatory practice staff around NCQA medical record documentation requirements and chart review process.
Develops and updates NCQA educational guides and materials.
Prepares presentation materials including PowerPoint slides screenshot captures tables and graphs.
Attends onsite practice meetings with practice consultant to present chart review findings and make recommendations for solutions to address deficiencies.
Photocopying electronic mailing receiving and sending mail scheduling meetings faxing and other administrative duties as required.
Will assist in project management activities to help keep projects proceeding efficiently.
