The care is delivered within PHHs philosophy policy and standards of community health practice along with the expertise of WOCN. Provide specialized education to patients and caregivers. Instruct clinicians in the specialized care of patients. Perform related documentation and supply management. Assist with evaluation of program objectives and participate in program improvement activities.Minimum Qualifications Bachelors Degree with minimum 2 years of nursing experience Current Massachusetts license as Registered Nurse Graduate of WOCN with certification within past 12 months and 1 year of recent home care OR CWCN (certified wound care nurse) and COCN (certified ostomy care nurse) with current certification 1 year of recent community health experience preferred Working knowledge of handheld patient care computer devices Strong interpersonal and customer service skills and ability to work with a diverse population Ability to communicate effectively in writing verbally and electronically Excellent organization and prioritization skills and ability to adapt to changing situations Ability to work as a member of an interdisciplinary team of health care providers Travel within local geographic area Physical EnvironmentPrimarily will work in patients homes. Potential exposure to blood blood products body fluids and chemical hazards.
