As an Advanced Practice Clinician (APC) at knownwell you will work alongside a team of committed physicians APCs registered dietitians registered nurses and registered pharmacists to provide comprehensive medical care with a specific focus on the diagnosis treatment and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.
Schedule: Monday Friday with flexible scheduling 32 patient contact hours minimum 8hrs of administrative time). Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone OnCall
EMR: Athena
OnCall: Phone call only in a rotation for one 1 week at a time. Average call volume <1 call per WEEK (as of 8.1.24. 24/7/365 physician back up.
Location: Hybrid in Plano TX
Responsibilities:
- Practice exceptional medicine within knownwell
- Sees adults for metabolic health and primary care services as isappropriate in a virtual setting. This may include sharing a panel with MD/DO other APCs of the local inclinic practices.
- May see pediatric patients ages 12 for metabolic health only based on training licensure and competency.
- Remote care using audio and visual technology to complete patient examinations evaluation assessments diagnoses and treatment plans.
- Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
- Coordinates with the broader care team to ensure the patient has the support they need.
- Proficiency with Microsoft Office applications recommended but not required.
- Documents as necessary to ensure accurate billing including review of remote patient monitoring data.
- Communicates with patients in a timely manner through our portal for questions and concerns that have been prescreened to necessitate a clinician response.
- Ensure patients feel known well
- Ensure your patient panel is open to any individual regardless of their current health status and comorbidities.
- Listen to the aspirations of the patient and ensure care is tailored to the patients unique health journey.
- Provide an environment in which patients feel comfortable and free from judgment.
- Offer exceptional encouragement as patients reach their goals.
- Coordinate across the collaborative care team
- Escalate any concerns to your dedicated site leadership team.
- Provide clinical guidance to medical assistants and nursing teams as assigned.
$130000 $150000 a year
