Reporting to the Primary Health Care Manager and Lead the Primary Health Care (PHC) Coordinator will work as a Practice Facilitator to provide practice support and advance the strategic priorities in PrimaryHealth Care Chronic Disease Management and Wellness and the Department of Family Practice portfolio. The PHC Coordinator will be focused on supporting reporting optimization efficiency and have responsibility to lead the planning ongoing development implementation and evaluation of services and project initiatives.
The incumbent works closely with Primary Health Care leadership providers teams and stakeholders to ensure localized support for planning coordination facilitation and evaluation of PHC projects/initiatives. The PHC coordinatoris responsible for developing and maintaining strong working relationships with health care staff provincial and other Zonal and NSH departments external individuals and partner agencies.
The Primary Health Care Coordinator will work collaboratively with family physicians and other health care providers and teams to support change management strategies that are inherent in the services plan for primary health care.
