Job Description
We are seeking a compassionate and dedicated Hospice Medical Social Worker to join our interdisciplinary team in Baker City. This role is pivotal in enhancing the quality of life for patients and families navigating terminal illnesses. The successful candidate will provide comprehensive emotional support counseling and guidance ensuring that both patients and their loved ones receive the care and resources they need during this challenging time.
Key Responsibilities:
- Conduct thorough assessments of the psychosocial needs of patients and their families.
- Collaborate with healthcare professionals to develop and implement personalized care plans.
- Facilitate access to community resources and support services.
- Offer bereavement support addressing emotional financial and social aspects of endoflife care.
- Serve as an advocate for patients ensuring dignity and comfort are prioritized.
- Coordinate treatments services and resources to improve patient quality of life.
- Provide counseling and referral services to help families cope with stress and hardships.
Qualifications:
- Masters Degree in Social Work.
- Proven experience as a social worker preferably in a hospice or healthcare setting.
- Strong interpersonal and communication skills with an ability to navigate complex emotional situations.
- Reliable transportation is required.
