This position will provide leadership and staff support for the training and education components of Population Health Management (PHM) activities and outcomes to Partners entities.

The Senior Program Manager in partnership with PHM administrative and clinical leaders and entity stakeholders will oversee the development and implementation of PHM training and education activities.

Responsibilities

Collaborate with PHM Medical Directors and Program Leaders in the development and implementation of PHM training and education activities
Collaborate with the Partners Continuing Professional Development (CPD) office to secure CME credits for PHM training and education events
Provide oversight and direction to the PHM training and education staff
Develop a road map for future training and education initiatives
Provide input on the communication of PHM program training and education efforts to ensure that they are effectively reaching the target audiences
Stay informed about national developments related PHM programs and skill sets
Consult with network leaders and practices about their needs related to PHM training and education.
Develop and maintain a detailed knowledge of Advanced Lean tools and methods NCQA Recognition and other key components of Population Health activities related to care management behavioral health integration and patient engagement. Promote this knowledge throughout central and local resources.
Work with Regional Service Organization (RSO) leadership in formulating effective training strategies for Medical Home transformation and continued improvement.
Effectively collaborate with the PHM programs to implement programs that are developed.
Develop and manage the curriculum for Partners PHM training and education services.
Develop and manage the materials used for Partners PHM training and education services.
Creatively develop training to meet the variable needs that exist (e.g. Webex sessions)
Oversee PHM training and education programs with support from other team members.
Identify and manage internal training needs for PHM staff.
Promote continuous improvement at Partners practices.
Promote continuous improvement within the central PHM training and education services.
Develop effective relationships and collaborate with network leaders physician leaders and Partners staff.
Respond to issues raised by stakeholders PHM staff and practices
Hire and manage staff in a way that promotes a supportive highperforming respectful environment that includes appropriate training appropriate delegation accountability and growth opportunities

BS/BA or equivalent required
Masters degree in healthcare administration public health preferred
At least 5 years experience in healthcare preferably in a provider setting involving training and education clinical process improvement and general project management

Management experience
Excellent communication and presentation skills
Strong organizational administrative and project management skills
Strong background in project planning and implementation of major initiatives
Ability to prioritize and resolve critical issues efficiently and effectively
Ability to work with all levels of an organization
Solid understanding of the health care environment
Strong project management skills
Influencing skills to engage leaders and clinicians
Experience with Microsoft Office applications
Ability to work independently and as a leader of a team
Ambulatory practice experience a plus
NCQA PCMH knowledge and experience a plus
Experience with Lean Six Sigma a plus
