Job Description

Position Description:

The Medical College of Wisconsins Department of Medicine Division of Nephrology is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. The Advanced Practice Provider (APP) works as part of a multidisciplinary health care team providing a broad range of diagnostic and therapeutic services for the patients of the inpatient services as a member of the Department of Medicine Division of Nephrology. The APP will have a strong foundation in evidencebased practice and ensure an environment of safe quality care. Located in Milwaukee WI at Froedtert Hospital and Outpatient Dialysis Centers.

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration patient care patient safety research quality improvement and teaching.

In this role you will be responsible for the following:

  • Collaborating with interdisciplinary health care teams in diagnosis and treatment of conditions requiring medical intervention in inpatient environment as a member of the General Nephrology service.
  • Writing accurate and timely progress notes procedure notes and/or admission and transfer orders.
  • Ordering performing and interpreting screening and diagnostic procedures and tests and appropriately communicating and documenting information.
  • Privileged to perform procedures beyond core as identified by the Nephrology services.
  • Facilitating and communicating with patient family and staff to promote continuity of care across the continuum of care.
  • Attending required training and meeting key regulatory requirements including those of the privileging institution CMS and the Join Commission.
  • Evaluating quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard.
  • Acquiring information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field through educational programs inservices professional associates and current literature.

Core Advanced Practice Responsibilities

  • Perform comprehensive histories and physical exams identifies normal and abnormal findings including physical cognitive and psychosocial status.
  • Order perform and interpret screening and diagnostic procedures and tests and appropriately communicates and documents information
  • Write accurate and timely progress notes procedure notes and/or admission and transfer orders
  • Develop pharmaceutical and nonpharmaceutical treatment plans.
  • Monitor effectiveness of plan of care
  • Educate patients on disease and treatment plans.
  • Perform procedures as trained and appropriately credentialed.
  • Document within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborate and/or consults with physicians and other health care providers as appropriate.
  • Educates counsels and assists patients and families regarding current health problems health maintenance activities and preventive behaviors.
  • Attend required training and meets key regulatory requirements including those of the privileging institution CMS and the Joint Commission.
  • Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field through educational programs inservices professional associates and current literature.

Preferred Schedule: Scheduling flexibility required to provide for an appropriate level of patient care coverage. Schedule Monday Friday with weekends on a rotating basis. Some night coverge may be needed.

Position Requirements:

Preferred Experience: Preference will be given to practitioners with prior experience in hospital cased care or medica/surgical patients specifically those with nephrology or dialysis experience.

Minimum Education:

Advanced Practice Nurse: Masters Degree or higher in Nursing
Physician Assistant: Bachelors Degree or higher in Physician Assistant Studies

  • Wisconsin RN License
  • Wisconsin Advanced Practice Nurse Prescriber License
  • Certification as Advanced Practice Nurse
  • Certification as Acute Care NP preferred.
  • Will consider certification as FNP and/or Adult Care NP
  • AHA BCLS
  • CITI Certification to be completed within 90 days of hire.

Physician Assistant

  • Wisconsin Physician Assistant License
  • Certification by NCCPA
  • AHA BCLS
  • CITI Certification to be completed within 90 days of hire.
