M/W and every other Friday at the Troy location and T/Th and every other Friday at the Warrenton location. Job Summary:

Integrates behavioral health services into clinic settings. Conducts patient interviews complete assessments and provide brief interventions and referrals to treatment. Counsels patients and family members and refer patients to family and community assistance agencies.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Provides decision support for treatment planning and assists in the implementation and monitoring of biopsychosocial treatment plans.
  • Counsels patients and family members through individual and family sessions by using brief solution focused therapy.
  • Counsels patients and family members through individual and group sessions to understand accept and follow recommendations given by their Primary Care Physician (PCP).
  • Works with primary care team to treat and manage patients with emotional and/or chronic physical health problems.
  • Develops interventions and sets goals for behavioral modifications.
  • Provides ongoing psychosocial and behavioral health outreach.
  • Develops collaborative relationships with community organizations in order to provide resources to patients for their various social needs.
  • Establishes and maintains relationships with psychiatrists to help comanage patients needing pharmaceutical interventions.
  • Works in a constant state of alertness and safe manner.

  • Performs other duties as assigned.


EDUCATION

  • Masters degree in Social Work


EXPERIENCE

  • Two years experience

PHYSICAL REQUIREMENTS

  • Frequent lifting/carrying and pushing/pulling objects weighing 025 lbs.
  • Frequent sitting standing walking reaching and repetitive foot/leg and hand/arm movements.
  • Frequent use of vision and depth perception for distances near 20 inches or less) and far 20 feet or more) and to identify and distinguish colors.
  • Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms malfunctioning machinery etc.
  • Frequent keyboard use/data entry.
  • Occasional bending stooping kneeling squatting twisting and gripping.
  • Occasional lifting/carrying and pushing/pulling objects weighing 2550 lbs.
  • Rare climbing.
