Job Description

Objective

To provide & coordinate medical services to ensure highest standards of patient care.

Principal Accountabilities :

Principal Accountabilities

(A) Strategic

Implementation of hospitals /medical services vision & philosophy.
Infection control Health care waste management awareness of patients rights & safety issues relationship building.
Supplement/take charge in case the vertical is on leave/absent etc.
Strategic inputs in her/his vertical & should forward recommendation for continuous improvement in the vertical.

(B) Operational

To prepare Discharge summaries for all assigned patients. Discharge summary shall be prepared and updated atleast a day prior to expected discharge date. Discharge Summary must be complete in all aspects with accurate diagnosis details hospital course procedures carried out significant tests and medications prescribed and follow up appointments. Once the discharge is confirmed by consultant ensure that the updated discharge summary is approved and signed by the consultant in eHIS.
To provide the best possible prompt medical care to all patients.
To work up all admitted patients to take detailed history.
To write progress notes of the patients in the records daily. The notes should be relevant to the patient current health status & should reflect the future plan in compliance with the admitting consultant after discussion with all concerned staff.
To liaise with the admitting consultants & to start the follow up treatments.
To coordinate all the references with other specialists if desired.
To ensure all the investigations are sent & the reports are interpreted & informed the concerned consultants. To ensure all related follow up including any change in the line of treatment on the basis of the reports of the investigations.
To give bedside over to the reliver while going off duty.
To ensure the diets of the patients are ordered as per the recommendations.
To ensure that the medicines to the patients are delivered as per the prescriptions.
To counsel all the discharged patients for further follow ups & to handover the discharge summary. The second copy of the same should be countersigned by the patient or the attendants & to be kept in the patient record file.
To take round with the Consultant & to document the instructions & to ensure their compliance.
To decide in liaison with the Primary Consultant about shifting patients to/out of the intensive care unit.
To provide help to nonICU team in managing sick/critically ill patients if possible.
Collective rounds with the verticals & to ensure all preventive & corrective measures to maintain the highest standards of patient care & service delivery.
Should facilitate all audits in the concerned areas & answer all their queries.
Inventory management In addition to that to ensure systems to avoid wastages/pilferages/thefts etc.
To ensure SOPs of the Medical Services be implemented.
To help the Functional & administrative heads in day to day operations.
To adhere to duty roster & to take prior permission of Functional & administrative heads in case of any mutual arrangements/ leaves etc.
To participate in the in service education programmes.

Performance Parameters

To prepare Discharge summary a day before actual discharge and submit to consultant for review by 7:00 PM
No error (or zero error) in Discharge Summary report
Submit approved final discharge summary by 10:30 AM in HIS system
Quality and timeliness of the service delivery of the patients.
Thorough history taking & Medical Records keeping.
To participate in quality audits in the concerned areas & to ensure preventive & corrective actions.
To ensure error free Medical care in all allocated areas.
To participate in the on going education.
To maintain highest standards in infection control & waste management.
To ensure complete workup of all patients and donors enlisted for Transplant.
To train all the subordinate staff for best practices to ensure implementation of service excellence programme.
Handling of grievances of patients & the internal staff.
To participate in the performance management system of the all reporting subordinates
Prompt grievance/complaint handlings.

Education

Essential: MBBS. Must be registered with State/Medical Council of India.

Experience

A Minimum of 2 years of relevant experience in the concerned specialty.

Knowledge

Quick Thinking Fast Moving and have wide spectrum of knowledge.
Strong communication skills.
Conceptual and Analytical skills.
Computer literacy and hands on PC.
