This position will float between the family medicine sites of Carolina Health Centers Inc. in the following counties in South Carolina: Abbeville Greenwood Laurens McCormick Saluda.
As a key member of our Carolina Health Centers Inc. team the physician will play a vital role in establishing and overseeing a comprehensive communitybased primary care program. The primary objective of this role is to enhance access to highquality primary care services focusing on familycentered care.
The physician will be responsible for designing and implementing an integrated delivery model that prioritizes prevention health maintenance health education and evidencebased chronic disease management. By collaborating with a multidisciplinary team the physician will contribute to improving health outcomes and promoting wellness within the community.
The ideal candidate will have a deep commitment to delivering patientcentered care and building strong relationships with patients and their families. Strong communication and interpersonal skills are essential for effective collaboration with colleagues and engaging with diverse patient populations. The physician should be wellversed in current best practices in primary care preventive medicine and chronic disease management.
