Hospice Clinical Liaison

As a Clinical Liaison heres a bigpicture view of what youll do:

  • Attends and participates in staff meetings educational programs and community events as requested.
  • Communicates identified needs and potential solutions to the VP of Hospice and to the Director of Hospice Clinical Services.
  • Explains hospice services and benefits to patients family members and significant others in a manner that facilitates an accurate and positive view of hospice services.
  • Obtains signatures on all required forms and provides copies to patient/responsible party.
  • Ensures all forms are completed dated signed and turned in to the Intake team.
  • Ensures all available medical records are turned in to the Intake team.
  • Establishes and maintains significant professional relationships.
  • Works closely with community hospital and facility education representatives to meet growth and development targets and maintain market acceptance and allegiance.
  • Facilitates the admission process to and from hospital and longterm carebased referral sources.
  • Assesses referred patients clinical needs and disease status and confers with patients attending physician and hospice physician regarding assessment to establish eligibility.
  • Facilitates patient discharge from hospital to home by establishing initial needs and coordinating transportation DME medication needs supplies and admission by the Interim HealthCare hospice case manager.
  • Oversees GIP patient admission process based on identified symptom management needs for that level of care.
  • Works closely with facilities and hospitals to identify transition and admit patients appropriately for general inpatient services.
  • Provides education to hospitals and facilities on the purpose process and goal of general inpatient care.
  • Performs other duties as assigned.
  • May have access to personal health information (PHI) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.

A few musthaves for Hospice Clinical Liaisons:

  • RN or LPN License is required (active and in good standing in the state of Virginia).
  • Current/Active CPR Certification is required
  • Two 2 to three 3 years of related experience required.
  • Hospital and/or longterm care clinical experience highly preferred.
  • Experience in healthcare marketing preferred.
  • Demonstrated effective written and verbal communication skills including the ability to communicate with all levels within and outside the company.
  • Demonstrated familiarity with home healthcare services referral sources and payors.
  • Computer and Internet literacy.
  • Demonstrated excellent organizational and detail skills.
  • Demonstrated ability to accomplish goals under short deadlines changing priorities and minimal resources.
  • Proven ability to work independently with minimal supervision.
  • Understanding of state and federal home health standards and regulations
