Under the supervision of the Life Enrichment Director the Household Coordinator is responsible for implementing and coordinating appropriate personcentered care programs and activities for our Assisted Living residents while maintaining the respect and dignity of each person. Facilitates a selfled team ensures compliance within the allocated budgets and emphasizes a high quality of care through appropriate team member models and increased involvement of daily life enrichment within the Household. The Household Coordinator ensures with the Nurse Leader that each resident receives daily nursing care and other services in accordance with the residents assessment choices Daily Pleasures and care plan.

ESSENTIAL FUNCTIONS:
Guides Household team in meeting residents needs and practicing routines based on person directed plans of care.
Plans and/or leads group and individual recreation programs.
Assists in transporting residents of Health Center to/from recreation programs and/or offsite activities.
Takes initiative in developing meaningful supportive relationships with residents of the Health Center.
Observes and reports any changes in or problems with residents.
Encourages participation by residents and engages the support of nursing staff for same.
QUALIFICATIONS:
Demonstrated Leadership skills and/or aptitude. Previous experience coordinating and implementing activities preferably in longterm care or health care setting desirable.
High school diploma or equivalent. College Degree Desirable.
Strong computer skills experience with MDS/Care plan and calendar creation software a plus.
Strong interpersonal skills to effectively communicate and collaborate with and respond to the instructions and requests from a diverse group of clients/ residents visitors and team members and to provide information with courtesy and tact.
Understanding of persondirected lifestyle principles including awareness of Residents/Patients/Clients/Members preferences and needs.
Ability to function independently have flexibility and demonstrate personal integrity when interacting with clients/residents families team members and others.
Ability to respond to and work with a mature resident population creating a warm caring and supportive environment for the enjoyment of the oncampus and off campus clients and to promote this philosophy to all team members.
Possesses an understanding of and support of the mission and philosophy of FellowshipLife Inc.
Availability to work a flexible schedule including weekends or during emergency situations or when another coverage cannot be arranged.
