About the role
We are seeking a dedicated and skilled Physiotherapist to join our team and make a difference in the community by providing essential services to clients in the Transitional Aged Care Program. Key responsibilities include:
Delivering physiotherapy assessment and treatment for clients in the communitybased Transitional Aged Care Program.
Managing cases for clients on the Transitional Aged Care Program to ensure their needs are met effectively.
Collaborating with community services health professionals and support groups to provide integrated care.
Actively participating in quality improvement initiatives within the Transitional Aged Care Team.
Providing education and training to the Transitional Aged Care Team and other community teams as required.
About you
We are seeking a dedicated Physiotherapist who is committed to making a difference in the lives of older adults. The ideal candidate will have:
Evidence of a Tertiary Qualification in Physiotherapy and current registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Physiotherapist.
A current NSW drivers license (Class C or above) with the ability to travel within the community to provide services.
Demonstrated ability to work both autonomously and collaboratively within a multidisciplinary team ensuring integrated and effective care for clients.
Strong communication consultation and interpersonal skills with the ability to engage with clients families and healthcare professionals effectively.
A commitment to ongoing professional development with evidence of leadership in quality improvement activities to enhance clinical practice and service delivery.
