Under the supervision of the Manager of Primary Health Care the Pharmacist will work with an interprofessional group of health care providers with collaborative family practice teams and primary health care chronic disease management teams and patients to provide patientcentered care that optimizes safe effective and appropriate drug therapy. This includes working on selected medical services identifying and resolving drugrelated problems completing medication reconciliation participating in interprofessional medical rounds providing patient education and making therapeutic recommendations to the team. The Pharmacist is also responsible to ensure safe and effective drug therapy by participating in and overseeing drug distribution systems (inpatient and outpatient) as well as contributing to the effective and efficient attainment of the goals and objectives of Nova Scotia Health Pharmacy Services.

Although the base site of this position is the Yarmouth Regional Hosptialthis position provides coverage to other locations within Yarmouth County as required.
