Home Health Physical Therapist (PT)
A Home Health Physical Therapist (PT) is needed in Prairie City OR.
Titan Placement Group invites you to explore an opportunity in Prairie City a small town known as a gateway to the Strawberry Mountain Wilderness and the Upper John Day River offering a hub for outdoor activities like motorcycle riding ATVs hiking and more with a historic downtown and scenic trails nearby.
Salary and Benefits
- The salary range is $40.15 $57.40 per hour based on years of experience

Responsibilities
- Monday Friday: Hours Depend on Shift (Open 7am 6pm)
- EMR: Cerner and WebPT
- Homebased care and Inpatient at an Intermediate Care Center
- Evaluate the care plans and administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function.
- 711 Patients per Day
Requirements
- Current And Active License in the State of Oregon or the Ability to Obtain
- Physical Therapist (PT)
- Masters or Doctorate Degree Required
- BLS Certification Required
