Job Description
JOB DESCRIPTION:
The MultiSpecialty Support Pharmacist is an advanced practice pharmacist responsible for assuring optimal drug use outcomes in high risk high cost or complex patient populations. She/He applies specialized knowledge of pharmacotherapy to direct patient care teaching and consultation. She/He participates as a contributing member of ambulatory care teams such as rheumatology neurology infectious disease dermatology and gastroenterology. She/He provides clinical pharmacy and medication expertise in each area including policy development pharmacy operations prior authorization support and medication therapy outcomes.
EDUCATION:
Doctor of Pharmacy and an ASHP accredited residency program or equivalent experience preferred. Licensed pharmacist in the State of Vermont.
EXPERIENCE:
3 years of advanced practice experience in their area of expertise preferred or equivalent experience in ambulatory care or a clinical outpatient setting.
