Reporting to the Manager Health Services the Case Coordinator will operate with a high degree of independence as a member of the multidisciplinary team. The incumbent is responsible for providing a collaborative clientdriven process that assesses plans implements coordinates monitors and evaluates the options and services required to meet the clients health and human service needs through the effective and efficient use of resources within Home Care Southern HealthSant Sud and the community.
The incumbent exercises the appropriate level of initiative and independent judgment in determining work priorities work methods to be employed and action to be taken on unusual matters. The position functions in a manner that is consistent with the mission vision and values; and the policies of Southern HealthSant Sud.
This term position may end earlier as outlined in your collective agreement.
