The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Health Resources Division Member Health Management Bureau.
The Health Resources Division seeks to provide health care for lowincome and disabled Montanans through Medicaid the Health and Economic Livelihood Partnership (HELP) the Health Montana Kids (HMK) and Healthy Montana Kids Plus (HMK Plus) Plans. The Health Resources Division is seeking a dynamic Primary Care Value Based Program Specialist to drive innovative valuebased and quality improvement initiatives for Montanas Medicaid programs. This position will play a pivotal role in designing implementing and managing programs that enhance primary care delivery and population health outcomes.
The ideal candidate will possess a deep understanding of valuebased care models quality improvement methodologies and the ability to propose creative solutions to complex healthcare challenges.This position is responsible for implementing managing and duties related to the Patient Centered Medicaid Home (PCMH) Program the Comprehensive Primary Care Plus (CPC) Program Passport Program and other innovative valuebased or quality improvement programs for Montana Medicaid.
