The Pharmacist works under the supervision of the Manager of Seniors Western Zone in collaboration with the Clinical Pharmacy Manager at YRH and acts as a member of the interdisciplinary Seniors Care Team providing service primarily in the emergency department and community setting. The Pharmacist works to provide assessments treatment plans and coordination of interventions which help promote the wellbeing of patients. The Pharmacist provides collaborative client centered care across the continuum supporting patients families populations and communities. This position includes considerable collaboration within the team and with community partners. Excellent interpersonal and written communication skills are required. The Pharmacist will help manage a busy caseload and use clinical judgement to prioritize interventions. The Pharmacist is responsible for adhering to Nova Scotia Health policies and procedures and to the professional standards of their licensing body the Nova Scotia College of Pharmacists.
