Location: Access Health/TriCounty Community Network 1481 Tobias Gadson Blvd Charleston SC

Shift: Day shift 8:00am 4:30pm with some evening responsibilities

Job Summary:

The Community Health Worker (CHW) will conduct a variety of outreach efforts throughout the Charleston Berkeley and Dorchester Counties with specific focus on rural communities uninsured adults and diverse communities. Represents Roper St. Francis Healthcare in the community to increase awareness and promote support for comprehensive sustained community programs designed and targeted to meet the primary health care needs of underserved South Carolina populations. This position will help patients and their families navigate and access community services other resources and adopt healthy behaviors. Develops new collaborative efforts and partnerships to advance the mission of health equity for the Community Health department. The CHW is also responsible for interacting and providing education to engaged patients. The CHW will follow an established plan of care designed to promote health status through patient selfmanagement skills. The CHW will be responsible for working with patients in the community setting and clinical locations. The primary focus will be on health education and social support navigation activities with the patient identifying resource needs and maintaining records of services provided.

Minimum Qualifications:

Education: High School Diploma or GED required.

Experience: One year of experience in a health care related setting or community agency is preferred.

Licensure/Certification: Healthcare Coaching Training/Certification must be successfully obtained within 6 months of employment for Home Health Positions and 1 year of employment for positions in Physician Partners Departments. Valid South Carolina drivers license with good driving record is required.

Knowledge/Skills: Demonstrates exceptional customer service communication and interpersonal skills. Must have good organizational and time management techniques. Demonstrates knowledge of community services and proficient with computer systems. Ability to function with minimal direction. Demonstrates knowledge of office equipment to include fax computer and phone.

Other: This position requires operation of a vehicle for Roper St. Francis Healthcare business. Due to this job requirement the selected candidate/employee must possess a valid current South Carolina drivers license. In addition the selected candidate/employee must be insurable under the Roper St. Francis Healthcare vehicle insurance plan which will be verified by reviewing licensure and driving record.

Contacts: Frequent interaction with patients/families physicians hospital/clinic personnel post acute care providers and community.

Work Demands/Environment:

Frequent bending stooping standing stretching walking and sitting. Requires full range of body motion including manual and finger dexterity eyehand coordination and safe driving skills. Requires frequent talking in person and on the telephone. Vision correctable to within normal limits. Specific visual requirements: visualization of documentation inspection of patient assessment of patient visualization of gauges. Hearing required (hearing aid permitted). Requires fine motor coordination operation of motor vehicle (or available transportation) operation of medical and office equipment or machinery. Other: access to a variety of clinical locations. Mainly inside. Occasional protracted/irregular work hours. Occasional exposure to unclean environments toxic substances hazardous and unsafe driving conditions and other conditions common to delivery of health care in the community.
