Position Title: MikmawClient Care Navigator
Location: HalifaxNova Scotia
Reports To: Provincial Manager Healthcare Operations
Term: FullTime Permanent
Date: 21st March 2025
I. Scope and Summary of Primary Responsibilities
The Client Care Navigator (CCN) coordinates access to vision rehabilitation services for Indigenous (and nonIndigenous) clients with vision loss.
Through community outreach the CCN ensures Mikmaw people with vision loss in Nova Scotia have access to culturally safe and clientcentered care informed by Indigenous perspectives expertise and Indigenous led collaboration and delivered in a manner that respects their cultures and contributions.
II. The role of a Client Care Navigator includes:
- Building relationships and partnerships with Mikmaw communities.
- Completing intake assessments with clients
- Scheduling VLRC appointments with service providers
- Monitoring and providing proactive followup for clients ensuring ongoing collaboration across services/providers.
- Ensuring all servicerelated activities provided are documented in the client health record following VLRC policies.

Knowledge and Skill Requirements
- Demonstrated knowledge understanding and application of cultural safety and competency within healthcare would be considered an asset.
- Strong understanding of Mikmaw culture customs and traditions.
- Knowledge of spoken and written Mikmaw language considered an asset.
- Knowledgeable about services available in Mikmaw communities.
- Knowledge of clientcentered philosophy related to interviewing assessments and scheduling practices.
- Excellent communication and interpersonal skills including shared decisionmaking and facilitation.
- Able to prioritize workload and manage competing tasks with excellent problemsolving skills.
- Proficient in technology (e.g.: Microsoft office case management and care coordination systems)
Experience and Education
- Registered health professional designation is an asset.
- Minimum bachelors degree in Social Work Gerontology or other related health care discipline.
