Schedule: 40 hours per week Monday Friday with every other weekend rotation 11:00am 7:30pm

What Youll Do in This Role:
The Home Health Aide (HHA) provides compassionate inhome care to PACE participants aged 55 assisting with activities of daily living (ADLs) homemaking and personal care to support independence. They collaborate with the Interdisciplinary Team (IDT) to follow individualized Life Plans promoting participants health safety and dignity while respecting their desire to remain at home.
Primary Responsibilities:
- Build trusting relationships with participants and families.
- Serve as a liaison between participants and the PACE interdisciplinary team for effective communication.
- Assist with personal care homemaking and respite to promote independence.
- Observe and report changes in participants conditions to the Interdisciplinary Team (IDT).
- Follow the participants Life Plan ensuring care aligns with their needs goals and preferences.
- Work closely with the IDT providing feedback to support participant wellbeing and plan updates.
- Help participants maintain functional abilities by encouraging safe daily task participation.
- Foster dignity respect and empowerment in all interactions.
- Accurately record services provided and report any changes to clinical or social work staff.
- Participate in team meetings and activities that enhance participants independence and quality of life.
- Adhere to all PACE policies procedures and regulations.
What Youll Need:
- High school diploma or equivalent (college coursework in social work human services or related fields preferred).
- Minimum 1 year of experience working with the frail elderly or longterm care populations.
- Active unencumbered CNA certificate in the State of Michigan.
- Reliable transportation and a valid Michigan drivers license.
- Current CPR certification (or obtained within 30 days of hire).
- Compliance with immunization requirements including updated vaccines.
