Job Description

PRIME LOCATION ACADEMIC PRACTICE ENVIRONMENT

Bruyre Health an Academic Health Science Centre excels in the provision of evidencebased health care and services for the vulnerable and medically complex with a focus on persons who require subacute geriatric stroke or palliative care. Among other programs Bruyre Health provides inpatient and outpatient clinical services in Care of the Elderly Complex Continuing Care Palliative Care Family Medicine and Physical Medicine and Rehabilitation. Bruyre is one of the largest health care centers of its kind in Canada and a resource for information on the special care required by the elderly people with chronic and terminal illnesses and persons having suffered loss of autonomy.

The Care of the Elderly Partnership at Bruyere is currently looking for LOCUM hospitalist coverage on Bruyres inpatent Geriatric Rehabilitation Service.

Work Expectations:

The services of a locum hospitalist physician with an interest in the interprofessional care of older adults living with frailty are needed on the Geriartric Rehabilitation Service. The provider will be joining a large collegial and supportive group to ensure the general medical care of patients admitted to this highintensity rehabilitation program with responsibilities including:

  • Admission assessments integrating a comprehensive geriatric approach
  • Ongoing daily medical care e.g. including diabetes management hypertension and infections
  • Regularly collaborating with other interprofessional team members
  • Actively contributing to weekly Interprofessional Team Rounds
  • Completing weekly medication reviews with the Pharmacist to optimize prescribing
  • Actively contributing to family conferences as needed
  • Preparing discharge prescriptions and completing discharge summaries on day of discharge
  • Daytime availability for regular visits and to manage emergency medical situations from 8AM to 5PM weekdays
  • Evening and weekend call shared with a roster of 1012 physicians

Remuneration:

  • Competitive pay
  • Funded through an Alternate Funding Plan

Reporting directly to the Chief of the Care of Elderly Department the successful candidate will have:

  • Doctor of Medicine (MD) Degree
  • Appropriate Certificate in medical practice from the College of Physicians and Surgeons of Ontario (CPSO) and/or College of Family Physicians of Canada
  • Registration in good standing with the CMPA
  • License in Ontario (or be eligible for licensure)
  • Excellent interpersonal and organizational skills
  • Previous experience/demonstrated interest in working with this patient population.
  • Bilingualism (French/English) would be an asset.
