Job Description
Hueman is actively hiring Nurse Practitionersto complete inhome Health Risk Assessments (HRAs) for Medicare and Medicaid members. The Practitioner will float between various clinics as far north as Stockton and south in Merced.Thisis a fulltime opportunity scheduled Monday through Friday from 8 am 5 pm. The providerwill work onsite at the clinic performing an average of810 scheduledexams per day.No oncall nightsweekends or walkins.

Position Summary
Qualified Nurse Practitioners and Physician Assistants interested inthispositionwill have the opportunity to interact with members in a oneonone setting supporting their comprehensive care team. Providers performing HRAs are not required to treat or prescribe.
Nurse Practitioner Responsibilities
- Perform comprehensive noninvasive physical assessments.
- Review medical history and ensure accurate and timely documentation.
- Conduct medical reconciliation to ensure patient safety and compliance.
- Provide basic patient education on wellness and preventative healthcare measures.
Nurse Practitioner Qualifications and Experience Requirements:
- Masters Degree OR commensurate experience and satisfactory completion of NPlicense
- Active State NPlicense
- Active National Board certification
- Current BLS certification
- 12 months of experience as a paid advanced clinical provider is preferred
