This position serves as a key liaison between patients families healthcare providers and community resources ensuring seamless access to care education and advocacy. The role is instrumental in improving health outcomes for children with complex medical needs by implementing strategic patient engagement initiatives identifying systemic barriers and driving program enhancements.
The Community Health Liaison provides expertlevel education on healthcare systems ensuring patients and families maximize available resources. Additionally this role fosters partnerships with healthcare providers and community organizations promoting the Childrens Clinics multidisciplinary care model to enhance referrals and access to specialized services.

Education:
Masters degree in the field of social work psychology or related subject area
Community Health Worker certification or relevant experience and qualifications to be certified in the state of Arizona as a community health worker.
Experience:
Relevant experience to qualify as a community health worker. Experience with both pediatric and adult populations preferred.
Licenses and Certificates:
Community Health Worker certification
FINGERPRINT CLEARANCE
Employees are required to maintain a current Arizona Department of Public Safety Level 1 Fingerprint Clearance Card. Evidence of a current and valid fingerprint clearance card must be received to begin and maintain employment.

The employee is occasionally exposed to fumes or airborne particles risk of electrical shock and risk of radiation. While performing the duties of this job the employee may potentially be exposed to infectious organisms during routine and emergency situations. The noise level in the work environment is usually moderate.
