Job Description
This project is in the proposal stage were awaiting to be awarded the contract
This position is in support of the County of Sarasota Florida to provide care for those with special needs who are displaced in the event of a largescale disaster. This position requires the individual to be able to respond to a disaster within 24 hours of notice and to establish care with in 72 hours of notification. This is a post disaster mission and will require the individual to be onsite until conclusion of the operation.
The Discharge Planner is responsible for coordinating and managing the discharge process for our residents ensuring a seamless transition from the shelter to their next level of care or placement.
Key Responsibilities:
- The Discharge Planner will work closely with healthcare providers residents and families to develop and implement comprehensive discharge plans that meet individual needs and comply with regulatory standards.
- Arrange and coordinate postdischarge services including home health care outpatient therapy and community resources.
- Communicate discharge plans effectively to residents families and staff ensuring clarity and understanding of next steps.
- Maintain accurate and uptodate documentation of discharge planning activities in compliance with regulatory standards and policies.
- Ensure discharge plans meet all applicable state and federal regulations including Medicare and Medicaid requirements.
- Provide education and support to residents and families regarding postdischarge care options and resources.
- Listen to patients regarding any health concerns they might have and report details to a Registered Nurse or Medical Provider (MD PA NP).

Qualifications :
Experience Required:
- Minimum of 2 years of experience in discharge planning case management or a related role in a healthcare setting preferably in special needs populations.
- Strong oral and written communication skills.
- Excellent interpersonal skills.
- Critical thinking skills.
- Cultural competency.
- Integrity and honesty.
- Experience with special needs populations (preferred not required).
Education Required:
- Bachelors degree in social work or a Human Services field included but not limited to Sociology Special Education Rehabilitation Counselling Psychology or
- Graduate from an accredited nursing program for Licensed Vocational/Practical Nurse located in a State the District of Columbia or a U.S. territory accredited by The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
Required Licenses / Certifications:
- Current full and unrestricted licensed as LPN/LVN in the state of Florida.
- Basic Life Support.
- Current and valid drivers license.
Required Knowledge Skills and Abilities:
- Understanding of Special Needs populations (Preferred).
- Strong knowledge of Medicare Medicaid and other insurance regulations related to discharge planning.
- Ability to navigate in an electronic work environment including electronic health records webbased training and communications.
- Knowledge of and moderate proficiency in common Microsoft Office programs specifically Microsoft Word Excel Outlook and SharePoint.
- Knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII).
- Demonstrate strong organization and time management skills.
- Demonstrate strong oral and written communication skills.
- Cultural competency and interpersonal skills.
Duty Hours / ONCall Requirements:
- This an upon notification position.
- Shifts will be 12 hours a day until complete.
Physical Demands:
- Physically capable of performing duties and responsibilities as listed above.
- Ability to work in an austere environment exposure to heat and weather.
- Frequent use of common office technology such as the computer telephone etc.
- Ability to lift carry and move up to 30 lbs.
- Ability to drive a vehicle on behalf of the company.
Travel / Rotation Requirements:
Travel to Sarasota upon notification.
Additional Information :
Pay range is based on several factors and may vary in addition to a full range of medical financial and/or other benefits. Final salary and offer will be determined by the applicants background experience skills internal equity and alignment with geographical market data.
Compensation Min: $24hr Max: $35hr
