Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older. ACTT provides intensive communitybased treatment support and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and be determined as requiring a high level of service needs.
ABOUT THE POSITION: Licensed Practical Nurse
Duties & Responsibilities
- Assist Psychiatrist or APRN (Prescriber) to prepare clients for their appointments
- Take vital signs and record all information in the medical record
- Document ongoing client progress in areas that involve side relevant side effects of medication (BP wt gain)
- Assist Prescriber in submitting routing and ensuring completion of lab work as ordered.
- Assist Prescriber with Medication Only clients to coordinate clientcentered treatment planning.
- Complete paperwork for authorizations benefits correspondence as assigned
- Communicate with primary care clinician as directed by the Prescriber re sharing of information
- Demonstrate competency in age related or specialty issues and developmental needs for each population served.
- Work with prescriber and family regarding referrals to other medical and treatment resources
- Maintains collaborative relationships with family members and other service providers including VNA and Pharmacists
- Provides individual and family consultation to aid clients in understanding and managing their symptoms
- Oversees provision of laboratory testing on site and off site according to agency policies. Triages returned lab tests as appropriate. Reviews for acute abnormal findings. Obtains screens.
- Provides injections for clients on injectable medicines and evaluates client and documents injection in the medical record
- Keep program First Aid supplies uptodate as per agency policy.
- Manage disposal of hazardous waste according to agency policy.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings supervision trainings and other meetings as assigned.
- Additional duties as required.
QUALIFICATIONS:
Education: Degree/Certification in nursing or in a related health care field preferred.
Licensure/Certification/Registration: Licensed Practical Nurse
Training: Training as a Licensed Practical Nurse

COMPENSATION*:
$22.7244/hr minimum and up commensurate upon experience
*Actual rates are determined at the time of offer and are based off of relevant experience as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or signon incentives. (Please note for parttime positions this range would be prorated based on hours worked)
