You will be updated with latest job alerts via emailPrimary Role: Coordinate and implement activities which provide opportunities for socialization education exercise relaxation and selfexpression aimed at fulfilling basic psychological social and spiritual needs of participants. The Activities Coordinator will assist the physically emotionally and cognitivelyimpaired individual as well as caregivers to explore appropriate recreation and leisure pursuits in accordance with best practices and funding requirements. Provides case management services to physical and/or medically disabled adults and elderly participants enrolled in the ADHC or SC program. Implements and monitors services listed on the care plan.
