Job Description
Essential Functions:
- Carries a patient census of 13clients as your schedule allows
- Evaluatesthe level of patient function and performance and equipment needs
- Developstheplan of care/treatment includinglong and shortterm treatment goalsinterventions and the intensity frequency and duration of services
- Completes initial and ongoing comprehensive assessments at appropriate time points
- Provides interventions and therapeutic treatment in accordance with the physician certified plan of care and occupationaltherapy professional standards of practice
Minimum Education & Experience Requirements:
- Florida Occupational Therapist License
- In the last five 5 years two 2 years of experience as a physical therapist
Knowledge Skills & Abilities Required:
- Able to provide proof of current CPR certification per organizationspecific policy
- Able to visually and aurally observe and assess the patient
- Able to effectively deal with changes based on patient needs and scheduling
Working Conditions & Physical Effort:
- Able to travel within geographic area serviced by office
- Work isperformed in a clients residence
- Able to work flexible schedule as needed
- Perform physical activity that may require lifting carrying pushing or pulling up to50 lbs. and physically stretching bending stooping twisting standing sitting walking reaching and climbing stairs
