Job Description

Thrive By 5 is currently hiring Bilingual Service Coordinators for the Early Intervention Program in the Bronx. This position involves providing continuous and interactive services that ensures the successful participation of eligible children aged 03 and their families in a homebased and/or communitysetting.

Essential Duties and Responsibilities:

The essential duties and responsibilities are not limited to the following:

  • Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP.
  • Ensure the IFSP outcomes and strategies reflect the familys priorities concerns and resources; and that changes are made as the families/childs needs change.
  • Coordinate the provision of all services that the child needs and/or are receiving.
  • Facilitate the timely delivery of services.
  • Continuously seek appropriate services and situations that benefit the development of the eligible child for the duration of the childs eligibility.
  • Perform related duties as required.

Qualifications Knowledge and Experience:

  • Bachelors Degree in health and/ or human services field.
  • Bilingual Spanish Bengali and/or Russian preferred but all languages welcome!
  • Ability to effectively communicate with clients and families via phone and email.
  • Previous experience preferred
  • Case management experience preferred
