Job Purpose: Ensures that all resident supports are provided in compliance with agency policies DDD (Division of Developmental Disabilities) regulations and Medicaid requirements. Provides a personcentered approach to supporting and counseling residents who are individuals with developmental disabilities in developing independent living skills. Residents may live in group homes or apartments located in singlefamily or multifamily homes operated by Our House Inc. This position includes being oncall for staffing and programrelated issues at the group home or supervised apartments.
Essential Skills and Responsibilities:
Supervisory
- Ensure direct care staff provide services according to residents goals objectives and behavior plans.
- Ensure medication administration follows Our House Inc. and Division of Developmental Disabilities (DDD) procedures.
- Meet with pharmacy monthly to review and approve medication deliveries.
- Supervise train and evaluate residential staff; conduct orientation assign tasks approve timekeeping build staff potential and administer discipline.
- Make staff promotion transfer and demotion decisions with the Area Director and HR.
- Serve as program oncall for staffing and program issues; provide coverage if needed.
- Manage staffing schedules documentation logs activities reports budgeting maintenance and resident meetings.
- Hold monthly staff meetings.
- Ensure residents hold weekly house meetings.
- Maintain home property and vehicle safety ensuring fire drills and addressing unsafe conditions promptly.
- Coordinate special training for residents with unique needs.
- Attend and organize inservice training per OHI and DDD guidelines.
Administrative
- Maintain residents daily records in the approved electronic system.
- Develop and update electronic client records and household files.
- Set annual residence goals with the Area Director and ensure timely completion.
- Meet with licensing inspectors and correct any deficiencies promptly.
- Manage residents funds ensure Medicaid compliance and follow budget guidelines.
- Attend monthly supervision with the Area Director and correct deficiencies as needed.
- Assist in revising policies train staff on updates and report personnel issues to HR.
- Attend monthly Manager Meetings.
- Develop resident goals with the Interdisciplinary Team via IHP ISP or ELP.
- Meet with DDD Case Managers quarterly for resident audits.
- Communicate with residents employers and day programs to track progress.
- Ensure compliance with residential regulations by tracking residents overnight stays.
- Submit monthly client attendance reports by the 1st of each month.
- Report and address maintenance needs promptly.
- Cooperate fully with inspections or investigations by Our House Inc. Medicaid and the Division of Developmental Disabilities.
Quality Assurance:
- Report all incidents to QA according to OHI and DDD policies.
- Responsible for the safety and wellbeing of the residents.
- Monitors residence to maintain compliance with Our House Inc. (OHI) and Division of Developmental Disabilities (DDD) standards according to OHI Standard Operating Procedures Manual and DDD Manual of Standards and Medicaid.
- Incorporates OHI philosophy in all aspects of job performance and instills that philosophy in staff through training.
- Monitors residents progress toward IHP ISP or ELP goals through completion of monthly reports.
- Identifies and meets special/individual needs of the residents such as physical psychological social religious civil legal and financial needs.
- Monitor staff performance and completes staff performance evaluations.
- Identifies community resources and assists residents in utilizing community resources.
- Acts as special advocate to individual resident(s).
Direct Care
- Transports residents in agency vehicle to appointments shopping recreational activities etc.
- Manages residents medical appointments in accordance with OHI policy.
- Observes residents for health problems and responds to medical needs. Sets or assists residents in setting medical/dental/psychological appointments with nursing staff regarding all residents medical statuses and appointments.
- Adequately provides support in all areas of adult daily living hygiene meal prep etc. when needed.
- Support and train direct support staff in appropriate direct care techniques based on the needs of the individuals in the home.
OnCall Responsibilities
- Work shifts as scheduled and when needed
- Respond to calls emails texts and other methods of communication regarding incidents and employee relations issues that occur at the home on the agency work phone (provided).
A valid drivers license with a good driving record
Minimum of 4 years supervisory experience.
Own transportation to and from work and between work sites
Minimum of high school diploma or GED equivalent
Must be 21
Must be able to attend and pass and maintain agency provided (Paid) trainings
Demonstrated ability to enter notes for services and billing in an electronic health records system
Intermediate to advance proficiency in Microsoft Office
