Job Description

Founded in 1875 our agency has grown over the years to address most of the challenges children adults families and communities face across the lifespan including substance use child abuse and neglect domestic violence/sexual human trafficking homelessness juvenile justice mental health neighborhood distress and poverty. Compensation: between $24.04 and $30.41 per hour (wage is determined by total years of relevant experience)
Work Schedule: Monday through Friday part time remote work maximum of 20 hours per week. Scheduling is flexible and to be discussed in the interview. This position does not require being on call or managing a crisis line

MINIMUM QUALIFICATIONS

  • Masters degree in counseling social work or marriage & family therapy required
  • Iowa licensed required (TLMHC LMHC LMSW or LISW)
  • Two years of experience preferred
  • Valid drivers license/ acceptable driving record

SUMMARY DESCRIPTION OF PROGRAM

Heartland Family Service operates a residentialbased crisis stabilization service Bridges Crisis Care. Bridges Crisis Care is a voluntary 3to5day stabilization program for individuals experiencing a mental health crisis that offers a safe secure environment with daily access to therapy medication management case management services and outpatient resources designed to stabilize the individual and assist them in returning to the community where they live.

SUMMARY DESCRIPTION OF WORK

This position functions as a fully integrated team member of the residentialbased crisis stabilization team. Diverse therapeutic responsibilities that include assessment crisis intervention individual therapy and goal planning are pivotal in this role; as well as ability to work with a diverse population of clients with cooccurring disorders utilizing evidencedbased practices. Decision making problem solving and highly developed interpersonal skills are critical.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • While working with a caseload demonstrates advanced clinical skills in diagnosis intervention formulation and implementation of stabilization plan utilizing a client centered approach.
  • Utilizes evidencebased practice models to provide therapeutic services to adults experiencing a mental health crisis.
  • Conducts an adult mental health assessment within 24 hours of crisis client program admission.
  • Establishes a stabilization plan with the client which specifies immediate treatment needs and followup.
  • Facilitates daily individual and/or group therapy sessions.
  • Maintains clinical records including client stabilization plan client progress updates therapy notes and a treatment summary meeting established standards of timeliness and completeness.
  • Provide support and consultation to team members and client family members.
  • Is dependable and punctual regarding scheduling and attendance.
  • Refers to other Heartland Family Service programs and other community services as necessary either concurrent with services or for aftercare.
  • Displays a courteous and caring attitude at all times to the clientele volunteers and visitors of the Agency.
  • Cooperates and collaborates with program area staff volunteers and other Agency staff.
  • Abides by all specific program and Agency procedures policies and requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
  • Creates maintains and shares an appropriate and dynamic selfcare plan.
  • Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agencys ongoing fundraising and friendraising efforts.
  • Essential functions of this job are to be performed remotely from home on the HFS remote worksite.
  • Performs other program related duties as assigned.
