Guardian Angel Senior Services of Amherst MA is now hiring an RN/LPN to do field supervisions for all PCAs HHAs and CNAs. Supervision will be provided throughout Franklin and Hampshire Counties.

Basic Function

Selfdirects home visits to maintain compliance with client assessment and care plan development caregiver placements and supervisions. Advocates for better health through interventions make recommendations as opposed to providing services. Completes required documentation. Ensures caregivers are following basic company protocols.

Requirements:

Drivers License

Own Personal Vehicle

Education/Training: Degree: RN or LPN

Experience:

Prior home care experience required.

Prior supervisory experience helpful.

Responsibilities:

Participate in creating daily client visit schedule based on needs and requirements. When cancellations occur proactively work with office managers to fill schedule gaps.

Schedule will be set up in 1 of 2 ways:

a. Block Time Nurse gives office availability for a period of time (for example 9a1p) and is paid for this block of time. Office is responsible for filling the time this includes time for documentation and travel.

b. Guaranteed Hours Nurse is paid 58 hour per day and shares responsibility for filling schedule with office staff. 4 visits a day for every 5 hours is the standard unless there is a visit a significant distance within the service area.

Complete and turn in all documentation and paperwork within 24 business hours.

Document according to contract and organization policy and procedures.

Report findings recommendations and concerns to care team accordingly.

Create review and update care plans for assigned clients. Communicate updated to caregivers as needed.

Act as liaison between contract nurses clients direct care workers and management.

Provide education training and coaching to caregiving staff during placement and supervision visits.

As schedule allows take on routine nursing visits as client needs dictate or managers request such as medication fills BP checks catheter changes etc

In urgent situations be willing to provide hands on personal care for clients in need.

Follow the same dress code as outlined for Caregivers.

Perform other related duties as assigned.
