Job Summary:
The Disaster Case Manager supports survivors by helping them navigate recovery. This role involves assessing unmet needs developing personalized recovery plans and coordinating services from various federal state and local resources. The Case Manager acts as a single point of contact for survivors and advocates for their recovery ensuring timely access to resources and services.
Duties and Responsibilities:
- Conduct indepth assessments of disaster survivors to identify unmet needs caused by the disaster.
- Collaborate with survivors to develop goaloriented recovery plans outlining the steps required for their recovery.
- Refer survivors to local state and federal services including housing food medical care financial counseling and employment support.
- Track the survivors progress toward meeting their recovery goals ensuring timely access to necessary services.
- Advocate on behalf of survivors to secure the assistance they need for recovery including housing and healthcare.
- Maintain detailed records of case management activities and submit regular reports to supervisors ensuring compliance with program requirements.
- Work closely with the Disaster Recovery Center (DRC) local agencies and nonprofit organizations to streamline services for survivors.
- Participate in all required training programs related to disaster case management HIPAA and related policies.
- Other duties as assigned by supervisor.
Qualifications:
- High school diploma or equivalent.
- At least 1 year of experience in case management social work or disaster recovery case management.
- Clean criminal background check.
- Strong interpersonal and communication skills.
- Must possess strong multitasking abilities attention to detail organizational skills and proficiency in documentation.
- Proficiency with computer and common office equipment as well as with MS Office products.
Preferred Qualifications:
- Bachelors degree in social work psychology sociology or other relevant behavioral science.
- Bilingual in Spanish and English (Speaking Writing and Reading)
- 1 year of experience in the provision of resources and referrals.
- 1 year of experience in providing services to families during crisis.
Physical Requirements and Work Conditions:
- Work is normally performed in a typical interior/office work environment.
- Work involves sitting and standing for prolonged periods
- This position will be located in Disaster Recovery Centers (DRCs) and may require occasional travel to impacted counties.
- The Case Manager may be required to attend large meetings or trainings in a centralized location.
- Flexible work hours required including evenings and weekends to meet the needs of disaster survivors.
