University Home Care is looking for a compassionate and organized Hospice Coordinator to join our team in Livonia Michigan as a Contractor Healthcare/Medical professional. As a vital member of our team the Hospice Coordinator will be responsible for coordinating and providing support to our hospice patients and their families. This individual will work closely with our interdisciplinary team to ensure highquality care and support during endoflife care.
Compensation & Benefits:
The Hospice Coordinator position is an individual contributor role with a compensation range of $17 to $23 per hour paid weekly.
Responsibilities:
Coordinate hospice care services for patients and their families in a timely and efficient manner
Collaborate with the interdisciplinary team to develop individualized care plans for each patient
Educate patients and families on hospice services procedures and options for endoflife care
Conduct regular assessments and evaluations of patients to ensure their physical emotional and spiritual needs are being met
Monitor and manage patient medications symptom management and pain control
Communicate effectively with physicians nurses caregivers and other healthcare professionals to ensure continuity of care
Provide emotional support and counseling to patients and families during endoflife care
Maintain accurate and updated patient records ensuring compliance with HIPAA regulations and documentation requirements
Participate in oncall duties as needed to provide support and services outside of regular business hours
Stay current on hospice care trends techniques and regulations through continuing education and training opportunities
Requirements:
Bachelors degree in Nursing or related field from an accredited university or equivalent experience (NOT REQUIRED)
Minimum of 2 years of experience in hospice or palliative care setting
Excellent communication organizational and interpersonal skills
Ability to work independently and as part of a team
Knowledge of Medicare and Medicaid hospice regulations
Compassionate empathetic and patientfocused attitude
Basic computer skills and proficiency in using electronic medical records systems office setting
Must be able to pass a background check
