The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI Juvenile Special Needs Diversionary Program) is responsible for providing personcentered case management services to youth diagnosed with severe and persistent mental health disorders who are currently in the SNDP probation caseload in Hays County.
The position reports to the TCOOMMI Program Director and requires the development and maintenance of effective professional working relationships with Hays County Juvenile Probation Officers and other community services providers. This is a MondayFriday nonexempt fieldbased position with flexible hours.
The TCOOMMI Juvenile Case Manager meets regularly with youth and families in their homes community probation court and Hill Countrys outpatient clinics to provide case management/skills training services for a caseload of 1215 youths. Services are intended to stabilize mental health symptoms reduce risk of recidivism increase the youth/familys awareness of and participation with community and natural supports develop selfadvocacy and assist with their ability to participate in independent behavioral health services. An elevated level of detailed electronic communication is required. Adherence to company and state guidelines is expected.
The person in this position must possess a passion and aptitude for working with youth who have severe emotional and/or behavioral disorders who are involved in the criminal justice system. Knowledge of psychotherapeutic medications intensive case management functions substance use treatment crisis services community resources and evidencebased practices in the mental health/criminal justice systems are required. This position necessitates innovative thinking patience resiliency skills in advocacy service coordination mental health assessment and evaluation collaborative personcentered recovery planning and dualdiagnosis treatment. The person in the position will effectively work independently and on a team prioritize needs during crisis situations and engage in regular meetings/training opportunities.
Youll definitely need:
Bachelors degree in the Social Sciences field from an accredited university.
Masters degree or college level courses in Criminal Justice or Social Sciences from an accredited university (preferred).
Previous experience working in the TCOOMMI program behavioral health criminal justice and/or case management QMHP certification; Bilingual (English/Spanish) (preferred).
A valid Texas State Drivers License
Current automobile insurance and the ability to travel
