Collaborates with multidisciplinary teams provides counseling and/or psychotherapy coordinates patient discharge planning complies with required standards for medical/legal responsibilities and serves as a resource to other allied and administrative staff within Outpatient Social Work and the Department of Care Management.
This position will support a growing neurology department working with outpatient stroke patients and caregivers participating in a variety of multidisciplinary clinics and providing case management services. Offering psychosocial assessment support resource connection and support groups to strengthen resiliency and recovery.
Requires a Master of Social Work (MSW) or Master of Science in Social Administration (MSSA) degree from an accredited social work program.Candidates must have an advanced knowledge of mental illness personality development knowledge of the types of treatment and skills in conducting individual group and family therapy. Must have knowledge of adjustment to medical diagnosis and chronic disease. Strong communication skills are needed to interact with patients patients family members consultants residents hospital personnel community agencies and other resources. Must also have psychosocial diagnostic skills and the ability to communicate effectively in both verbal and in written form.Licensure (LCSW) to practice independently at the clinical level as required by applicable state requirements.
