Per Diem Pharmacist Greenlawn Long Island New York OFFS

Job Title: Per Diem Pharmacist

The Per Diem Pharmacist serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.

Roles & Responsibilities:

  • Dispenses medications accurately and in a timely fashion.
  • Resolves questions regarding formulary and nonformulary medications following policies and procedures.
  • Provides dosing and therapeutic recommendations that take patient age/kinetic based on literature.
  • Performs clinical chart reviews on a scheduled basis.
  • Performs Patient Care Unit inspections on a scheduled and timely basis.
  • Prepares parenteral medications using sterile aseptic technique.
  • Maintains adequate medication supply in the Pharmacy area and informs supervisor of impending shortages
  • Intervenes and documents interventions to correct/ improve pharmacotherapy.
  • Reports adverse drug events and medication errors occurring in area serviced.
  • Takes responsibility for the operation of the assigned Pharmacy area and activities of all supportive personnel.
  • Maintains pharmacological knowledge by reading professional publications attending workshops conferences and C.E. programs to maintain professional competency.

Requirements:

  • Graduate from an accredited College of Pharmacy (ACPE).
  • 1 year minimum as licensed Pharmacist
  • Licensed Pharmacistpreferred
