YSS is looking for an organized observant and outgoingIHS Care Specialistto join our Family Life Center team!
The person in this position serves as a member of an interdisciplinary team for Integrated Health Services (IHS) and plays an active role in designing care plans for children and their families. As the first point of contact for clients Care Specialists are primarily responsible for completing assessments making and tracking referrals and providing followup and coordinating with internal team members and other professionals. Additional responsibilities include:
Use data (assessments evaluations observations) to identify health/wellness needs and support systems.
Facilitate care planning and goal setting developingcrisis plans when needed.
Educate clients/families/advocates on wellness medication and behavioral health.
Schedule and coordinate appointments (initial crisis intake) with/for clients and provide transportation to appointments as necessary.
Complete required reports and documentation.
Coordinate outpatient funding including EMRinternal funding coordination credentialing and compliance with thirdparty payers.
Demonstrate flexibility in duties and schedule to meet client needs and benchmarks.
Attend meetings and training sessions to evaluate service outcomes improve service qualityand enhance team performance.
Assist with special projects as assigned.

MUST HAVES Required Qualifications
A bachelors degree in social work or a related human services field.
Two 2 years of experience working directly with mental health and child welfare. Additional years of work experience may be substituted for education.
Travel including transporting clients is required for this position. For YSS insurance purposes you must:
be 21 years of age
have a valid stateissued drivers license
have a clean driving record and
have adequate insurance coverage per YSS policy.
SHOULD HAVES Preferred Qualifications
Proficient in basic computer skills using Microsoft Officeproducts (Excel Outlook Teams Word) and navigating multiple systems including Electronic Medical Records.
Strong communicationskills including observing listening speaking and writing; able to effectively communicate with clientsthe publicand other staff members.
Experience in a clinical setting providing direct support to patients andtheir familiesfrom diagnosis through treatment.
Passion for helping others and ensuring customer satisfaction in every interaction.
Ability to:
work independently take initiative manage time effectively and prioritize tasks to meet deadlines
think critically solve problems and make sound decisions
establish and maintain positive appropriateand collaborative working relationships and
manage resources effectively.
