Home Care Licensed Practical Nurse (LPN):Enjoyindependence autonomy and flexibilityin the field to visit patients one on one in their home environment.

Hours: Fulltime Days 8:00am 5:00pm
Territory:This position will work in the field helping patients in their home setting. Our office is in Roseville MN and the service area of where our patients live is in the 11 County Twin Cities Metro Area. The general territory for this position would be the Northwest Metro area.

A few musthaves for this position:
- Current license as an Licensed Practical Nurse (LPN) in the State of MN.License is active and in good standing.
- At least 1200 hours of experience as an LPN/LVN or completion of the Offices preceptor program.
- 2 years experience as an LPN is preferred.
- Able to travel within geographic area serviced by the office.Valid drivers license reliable transportation and proof of auto insurance.
- CPR Certification
Some functions of what you would do as an LPN:
- Participatein ongoing interdisciplinary assessment of the patient.
- Participatein the development and evaluation of the plan of care in partnership with the patient representative and caregiver(s).
- Provides services that are ordered by the physician as indicated in the plan of care.
- Recognizes and reports changes in the patients condition to the RN.
- Counsels and educates the patient and family in meeting healthcare related needs.
- Completes timely and accurate clinical notes including addressing patients progress.
