Rotating 08 Hour (United States of America)Join our team! Stanford Health Care Department of Pharmacy and the Stanford Medicine Cancer Center are teaming up to launch a new clinicbased ambulatory care clinical pharmacist position in Hematologic Malignancies. The clinical pharmacist will join the multidisciplinary clinic to support patient care with medication management and optimization. Changing the Future: Stanford Medicine leverages its resources as a premier academic institution to develop cuttingedge diagnostics and treatments providing patients with the highest quality care. The Cancer Destination Service Line is a National Cancer Institute (NCI)designated Comprehensive Cancer Center that serves patients worldwide. Its mission of treating the whole patient through comprehensive care navigation and supportive service programs addresses the entire patient experience through diagnosis treatment recovery and beyond. You will be at the forefront of the ongoing innovations and initiatives that improve how Stanford delivers care and improves the lives of the patients it serves.
Clinicbased Ambulatory Care Oncology Pharmacists in this department have opportunities to:
o Participate new processes and establish the workflows
o Support medication management under a collaborative practice agreement
o Provide comprehensive medication review and reconciliation
o Provide patient education and teaching

A Brief Overview
The Hematology Oncology Clinical Pharmacist provides the full range of distributive clinical consultative research investigational and/or administrative pharmaceutical services. Incumbents select compound manufacture purchase dispense and/or preserve medicines and other therapeutic agents; serve as a member and consultant on treatment or diagnostic teams; disseminates drug information to members of the hospital staff and interested community groups; may participate in hospital research projects or perform pharmaceutical research.

What you will do
- Clinical Pharmacy:
- Applies knowledge of solubility behavior of substances stability of usages of and limitations on modes of administering and similarly applies knowledge of incompatibilities in evaluating dosages and permissible concentrations of and in anticipating preventing and treating undesirable chemical reactions.
- Applies knowledge of the physical and chemical properties and characteristics of computing dosages weighing and measuring and mixing and compounding and chemicals.
- Has legal responsibility for proper interpretation of prescriptions and the dispensing of prescribed products and call to physicians attention any synergistic antagonistic and cumulative effects as well as questionable overdoses or incompatibilities.
- Identifies fine differences between as they relate to therapeutic efficacy of the products.
- Maintains continuous awareness f new developments in the fastmoving drug field and be aware of emerging patterns systems and techniques in pharmacy practice.
- Maintains records documents patient data prepares written reports and provides oral reports concerning patient care.
- May contribute to the accredited teaching programs indoctrination and refresher training courses.
- Specifies proper storage protection and preservation of therapeutic agents in accordance with the standards of the United States Pharmacopoeia (U.S.P. and the National Formulary (N.F.
- Investigational Drug Service:
- Provides assistance in the coordination and maintenance of pharmacy services relating to research at SHC.
- Participates in sponsor site qualification and initiation visits.
- Supports practices relating to pharmacy dispensing based on IRB approved clinical trials.
- Provides guidance to investigational drug service pharmacy technicians.
- Upon request attends various meetings to provide pharmacy input on drug protocols.
- Serves as the liaison between the study team protocol sponsor and nursing to coordinate drug therapy.
- Participates in audits from the sponsors FDA DEA NIH and other regulatory agencies as needed.
Education Qualifications
- Bachelors in Pharmacy from an accredited college or university.
Experience Qualifications
- Two 2 years of progressively responsible and directly related work experience.
- One 1 year if completed a PGY1 residency.
Required Knowledge Skills and Abilities
- Ability to apply judgment and make informed decisions.
- Ability to communicate effectively both orally and in writing.
- Ability to foster effective working relationships and build consensus.
- Ability to plan organize prioritize work independently and meet deadlines.
- Knowledge of influence of patients age sex concomitant disease states concurrent drug therapy foods and diagnostic procedures on drug activity and disposition.
- Knowledge of local state and federal regulatory requirements related to areas of functional responsibility.
- Knowledge of Medical terminology and abbreviations; pharmacology and pharmacological nomenclature; medical therapeutics pharmacokinetics and disease states.
- Knowledge of methods and procedures involved in proper handling and storing of pharmaceutical supplies.
- Knowledge of physiology anatomy pathology chemistry and mathematics as they relate to pharmacy practice.
- Knowledge of prescription and nonprescription and drug products and their indications efficacy dosage mechanisms of action; fate and disposition (if known) major contraindications and potential side effects.
Licenses and Certifications
- RPH Registered Pharmacist required and
- BLS Basic Life Support required within 30 Days or
- ACLS Advanced Cardiac Life Support required within 30 Days

Base Pay Scale: Generally starting at $72.55 $96.15 per hourThe salary of the finalist selected for this role will be set based on a variety of factors including but not limited to internal equity experience education specialty and training. This pay scale is not a promise of a particular wage.
